CHADIS

The SEEK PQ-R and the SEEK PQ-R Plus (with follow-up probes to clarify an issue) are available on CHADIS (Comprehensive Health and Decision Information System) and can be accessed via any web enabled device. The PQ-R is available in English, Spanish, French, Vietnamese and two dialects of Chinese.

CHADIS is an online clinical process support system for gathering a wide variety of pre-visit information from parents and patients. It provides their responses to health professionals at the start of the visit, linked to optional decision support. The pre-visit data and some clinician decisions trigger printable patient specific education in a website for each child (MemoryBook or CarePortal).

SEEK, CHADIS and EHRs

CHADIS can be used with or without an EHR. If using an EHR, the information can be copied and pasted to any EHR. Several EHR’s are integrated with CHADIS and results are automatically entered.

In addition, the SEEK PQ-R Plus, an adaptation of SEEK, has follow-up questions for positive screens to gather important clarifying data. The approach uses motivational interviewing (MI) while probing the impact of a problem and barriers to engaging in help. SEEK PQ-R Plus provides ‘in the moment’ decision support with a monitor serving as a teleprompter. In addition, individualized parent handouts can be printed and/or stored in a child’s MemoryBook. A simple “refer button” links families to appropriate local resources. MOC-4 credits are available for pediatricians, family physicians, internists and physician assistants implementing SEEK via CHADIS.
